Certificate of Occupancy/Business License Application


On the following pages, you will be prompted with a series of questions related to the Certificate of Occupancy and licensing of your business.

A PDF application will automatically generate upon the completion of these questions. You will be prompted to provide an electronic signature at the bottom of the PDF in order to submit the application. Note: Unused fields may appear in yellow but should be left blank.
If you find that you need to make a change/update to the populated PDF, use the BACK button on your browser to make your changes. You will need to reattach any documents. A new PDF will populate when completed.

If any of the conditions in bold below apply to your business, please have the corresponding documents available in electronic format to be attached to this application, when prompted.

Licensed by State of Illinois - State of Illinois Business License

Once the document has been submitted, Village of Schaumburg Staff will review your application/attachments to determine applicable fees. You will then receive an email (sent to the Business Email Address that you provided on the application) that will include your fee total owed. At the appropriate time, you will receive a payment link to provide payment for applicable fees.

1. General Business Information

This section asks for information about the local business that will operate in Schaumburg. This information will be used to create your Certificate of Occupancy and your Business License.

We ask for local contact information to keep on file in case we have questions or need to reach someone at the business. 

Is IL Sales tax collected?

You have identified that Sales Tax is collected at your business.

2. Corporate Information

This section asks for information about your legal corporate name, if there is one. The corporate name may be different than the trade name or DBA of your business. This information will also be used for your Certificate of Occupancy and your Business License

We ask for a corporate contact information to keep on file for legal or adjudication matters.  If there is no corporate organization, please type “Same as DBA” in fields below.

3. Applicant Information

This section asks for information about whoever is filling out this application. This may be the business owner, a property manager, a landlord, or any other representative of the business.

The applicant will receive email confirmation when this application is received, when it is reviewed, and when the fees are approved by all departments.

4. Billing Contact Information

This section asks for information about where to send annual Business License Renewals. This may be someone at the local business or at the corporate address. We want to make sure that any renewals or invoices are directed to the correct person.

This person will also receive all annual Business Licenses

Supplemental Questions

A Certificate of Good Standing is a document issued by the Illinois Secretary of State showing the official existence of registered business entities like a Limited Liability Company (LLC), Corporation, LP, LLP or Not-for-Profit that is authorized to do business in Illinois.

Additional Information

We will need to inspect your business before we issue your Certificate of Occupancy and your Business License.

Inspections are performed Monday through Friday only. Please choose a date at least two business days from when you submit this document, and enter it in the space below.
