CDBG CV-19 Small Business Recovery Grant 

Phase 2

The Village of Schaumburg CDBG-CV Small Business Recovery Grant is now open to additional business types and contains revisions to the eligible use of funds. Applications will be reviewed and awarded on a first-come, first-served basis until all funds are disbursed. 

Please complete the steps below before proceeding with your application. Once you start your application you cannot save it and return later. You will need to start over if you leave before you hit the submit button.

Step One  Read the program guidelines to understand the program.

Step Two:   Have these items ready to upload in the application portal:

  1. 2019 Business Tax Return
  2. 2020 Business Tax Return
  3. 2021 Financial Statements
  4. Copy of signed Lease or Proof of Property Ownership
  5. Most recent IRS Form 941 
  6. Valid Identification-Drivers License, State ID, or Passport

Step Three: Download these Village documents onto your computer, fill them out, and then have them ready to upload once you enter the application portal.

 8. Full Time Equivalent Payroll Statement

 9. Duplication of Benefits Form for each assistance grant & loan received

10. Signed W-9 IRS form

Certification

I agree to comply with the program regulations of the U.S. Department of Housing and Urban Development (HUD), Community Development Block Grant-CARES ACT (CDBG-CV), and the Village of Schaumburg, including 24 CFR Part 570-Communities Development Block Grants.

I acknowledge that Village staff will ensure my for-profit business has a current Certificate of Occupancy and has no license or fee delinquencies. I confirm my business is not a 501 (c)(3) nonprofit organization.

I am aware that Village staff may request additional information, including but not limited to Small Business job creation/retention certifications from employees.

I certify that the expenses for which I am seeking reimbursment are related to the prevention, preparation for, and response to COVID-19, and will repay to the VIllage of Schaumburg any expenses that have been determined to have been paid for using other assistance received or if I no longer meet the obligations stated in the program.

I am aware that I must sign a Subrecipient Agreement with the Village of Schaumburg to comply with HUD regulations and to enable payment to be released to my business.

I hereby declare under penalties of perjury that all information submitted is, to the best of my knowledge accurate and true. I understand all guidelines and requirements of this application. The property, business, and employees are free and clear of any default or violations with any Federal, State, or local departments and programs including but not limited to any HUD and/or CDBG funded programs, property code compliance, permits and zoning compliance, food and beverage tax, etc.
